Refer a Client

The Georgia Rehabilitation Services Volunteer Partnership (RSVP)  provides outpatient rehabilitative care one time a month that maximizes independence, wellness, and community participation for underinsured or uninsured people  with a diagnosis of stroke, brain and spinal cord injuries.  There will be COVID precautions and once accepted clients will be notified.

Admission to the GA RSVP Clinic is as follows:

  1. People with a diagnosis of acquired brain injury (traumatic, non traumatic or stroke) and/ or diagnosis of a spinal cord injury (traumatic or non traumatic). 

  2. Age 15 or older 

  3. Uninsured or underinsured for rehabilitation services

  4. Up to 3 years post discharge from an acute care hospital

  5. Meets Federal poverty guidelines 2023 Federal Poverty Guidelines

Our Admission Coordinator will contact you about the referral and give you information on how to fax the following medical records:

  • Admissions History & Physical by MD

  • Injury Related Imaging Reports

  • Consultation Reports

  • Recent Therapy Evaluations and Treatment Notes

  • List of Discharge Medications

  • Discharge Summaries - Physicians, Therapists, Case Manager/Social Worker
